with store name and dated between October 30th, 2011 and January 31st, 2012 with purchase price(s) circled (photocopies acceptable).
4) Cash register receipt(s) from Kimberly-Clark purchase imprinted with store name and dated between
October 30th, 2011 and January 31st, 2012 with purchase price(s) circled. Receipts from multiple retailers and transactions allowed (photocopies acceptable).
